Our cemetery grounds have a Veterans Circle where military services can be held and military plaques can be displayed, a flagpole columbarium for the above ground burial of cremains, and inground burial sites.
The preferred method is the DD Form 214, Certificate of Release or Discharge from Active Duty. If the DD Form 214 is not available, any discharge document showing other than dishonorable service can be used. The DD Form 214 may be obtained by filling out a Standard Form 180 and sending it to:

Ponte Vedra Valley is available 24 hours a day, seven days a week. Our office is open Monday through Friday from 9 am to 5 pm, and Saturday from 9 am to 2 pm, with appointments available on Sunday and after hours.
